On an SMP-400 system, the system was unable to clear the electronic level UART to the interrupt in the interrupt controller chip on the processor board.

What you should do:
Note the error data that is displayed on the SMP-400 internal diagnostic LED.
If the error reoccurs, contact your Giddings & Lewis service representative.
